ICS 237 CG Incident Mishap

The ICS 237 CG is a USCG form designed to record incident mishaps for Coast Guard personnel during a response. This form is used only when directed by the incident Safety Officer. Injured CG Person’s Information

This section contains information about the person injured and the narrative of the mishap.

First, enter the Date/Time, Location, including coordinates. For more information see the Edit Location page.

Second, enter the CG Unit, OPFAC, and select name of injured person from the drop down or use the Create New button Picture 4 to add a new person.

Next, type in the age, select the Sex, Rank, and Injured Category from the drop down.

Then, type in the narrative of the mishap in the notes filed.

Note: To enter Rank/Rate/Grade for military personnel click on the Star button Picture 9 to lookup a rank.

Mishap Details

This section contains more detailed information about the nature of the injury and any damage to the specific property.

Select the part of body injured (if applicable).

Select the nature of the injury and document any days hospitalized, work days lost, and/or days restricted.

Select the personal protective requirements required/utilized.

List any damaged property details and type(s).

Finally, complete the form by entering the name of the person completing the form and the report number issued for this mishap.

Click the Save button Picture 6 once the form has been completed.
